Shin Splints (Medial Tibial Stress Syndrome)
A shin splint is a painful condition that occurs from damage to muscles along the shin, and the pain can occur anywhere from just below the knee to above the ankle. There are two major types of shin splints: posteromedial and anterolateral.
Common Causes of Shin Splints
The general cause is continued, repeated stress to the lower leg. This can occur from a variety of factors, the most common being:
- An inexperienced runner just beginning to run (too much too fast).
- Running on side slopes (ie. banked tracks).
- Tight Calf Muscles, exerting extra force on shin muscles.
- A sudden change from soft to hard running surfaces.
- Poor or worn out footwear.
- Excessive uphill running.
- Poor running mechanics. This could include:
- Heavy forward lean
- Excessive weight on the ball of the foot
- Running with toes pointed outward
- Landing too far back on the heels causing the foot to flap down
- Overpronation - an image example of overpronation is shown in the image on the right
Of all of the possible causes, pronation (foot roll) is the most likely to be overlooked.
...A Bit of Information About Pronation
When one talks about pronation, they are talking about the side-to-side motion of the foot while the body is walking. How the weight is transferred from heel to toe when walking or running is what is studied and often analyzed to isolate potential causes of injury (current or future) especially in athletes that run/sprint. When the body is immobile but standing, pronation refers to the amount of inward roll of the foot toward the arch. Significant pronation (also termed as overpronation) is more likely to occur in people that are flat footed.
If you are suffering from overpronation, not only are you more likely to get shin splints - you are also at increased risk for contracting:
- heel pain
- plantar fasciitis
- illiotibial band syndrome
- stress fractures of the lower leg
- lower back pain
- patellofemoral pain syndrome
- achilles tendonitis
Posteromedial Shin Splints
Posteromedial shin splints affect muscles in the back and inner part of the lower leg. These muscle groups are responsible for lifting the heel to support the runners weight on the ball portion of the foot when running.
This type of shin splint is often caused by running on a banked track or non-level running surface or wearing improper shoes that do not protect the foot from rolling (pronation).
Pain begins on the inside of the lower leg (usually within 7 inches above the ankles), but will worsen and continue to rise up the leg.
Initially, only tendons of the muscles will become inflamed, but if running continues, the muscles themselves could become affected. At the worst, the tendon could become detached from the bone - a painful occurrence that causes bleeding and excessive inflammation.
To allow a posteromedial shin splint to heal, the runner must temporarily stop running and do other kinds of exercise recommended by their doctor or physical therapist.
Special shoes may be prescribed during the healing phase, and it may be advisable to look into potential problems with pronation of the feet. This can often be solved by researching and using shoes that better prevent pronation and/or the avoidance of running on side slopes.
Anterolateral Shin Splints
Anterolateral shin splints affect muscles in the front and outside parts of the shin, and is the result of a natural imbalance in the size of opposing muscles.
Shin muscles pull the foot up, whereas the large and powerful calf muscle pulls the foot down every time the heel strikes the ground when running. These calf muscles can exert a lot of force, enough force that can actually injure the muscles around the shin.
Anterolateral shin splints will cause pain in the front and outside of the shin. Initial pain is felt when the heel strikes the ground though eventually the pain just stays constant.
To allow this type of shin splint to heal, the person must stop running and do other kinds of exercise recommended by their doctor or physical therapist. Such exercises usually involve stretching the calf muscle, as tight calf muscles put a lot of pressure on the shin muscle and tendons.

First - Get To A Doctor For A Proper Diagnosis -
Why? You May Be Suffering From A Stress Fracture
A stress fracture is often mistaken for shin splints and they are not the same! Stress fractures are small cracks (and/or severe bruising) in bone and are caused by overuse or repeated impact - both which are common in the foot and lower leg.
Stress fractures are common in athletes and military recruits
To begin with, your doctor will gather a medical history about you and your current condition and symptoms. He/she will ask about the amount of pain you are having in the leg and questions related to the duration of symptoms and range of motion loss should be expected. Further details about what you suspect initiated the issue, when it started, and whether or not you have ever had treatments for this or a similar condition in the past are very helpful in assessing your injury. This will help the doctor to determine if you have a more complex injury in the area and rule out any other injuries that may have once been symptomatic.
The doctor will probably poke and prod along the shin area while looking for evidence of bruising as this is one of the primary ways to distinguish a shin splint from stress fracture. If the physician is satisfied that there is definitely an issue, they will probably order An X-Ray of the area. Further to this, an MRI may be considered as well as stress fractures can be very tiny and it is generally accepted that MRIs can provide greater specificity in a problematic area.
Tendon and muscle injuries in the lower leg are generally slow to heal, as it can be difficult to give your legs the rest they need. The goal is to rest and protect your injured soft tissue and achieve a fast recovery to minimize re-injury so you can get back to normal quickly.
It is important to treat shin splints as soon as possible, as any activity or strain you put on your calf muscles and tendons can re-injure it further. Though the pain, discomfort, and inconvenience of shin splints can be overwhelming, it is possible to overcome it. Stage 2: Focus on Increasing Blood Flow To The Injured Tissue - As This is The ONLY Way It Will Heal For the Long-Term
Even though the concept is simple, improving blood flow to injured tissues can be difficult. Traditional methods require your muscle to move to promote blood flow (exercise), but that same motion that promotes blood flow can at times lead to making your pain and condition worse.

When people think blood flow beyond exercise, they instantly think "heating pad". Unfortunately, many individuals make the mistake in believing that a simple heating pad or microwavable gel packs will provide the blood flow needed to heal injured tissues. Nothing could be further from the truth.
To begin with, heating pads and warmed gel packs work on conduction. The heat they generate has to be forced through multiple layers of skin and fatty tissues. With each layer of skin the heat passes through, there is a corresponding energy loss because that heat is absorbing into each layer it passes through- the skin is an excellent insulator! Your body responds to this heat by increasing the blood flow near your skins surface (not in your injured tissue) to dissipate the heat and it does so quite effectively.
By the time what little heat reaches below the skin, sub-dermal layer and fat layer into the targeted area (the dense, injured tissues), very little energy remains and this does not effectively enhance blood flow. With little blood flow reaching these injured tissues - healing just doesn't happen.
The heating pad may feel comforting when on the body, but that is because the warmth is absorbing into the skin. Moist heat has been believed to be more effective at warming internal tissues versus dry heat, because it is believed that water transfers heat more quickly than through air. Clinical studies do not support that popular belief.
When you are using a heating pad, all that you are really accomplishing is warming of the skin. The unintended consequence is that you draw blood flow AWAY from the targeted tissues (the injured ones deeper in the body) and direct the blood flow to the surface of the body.
Warming the skin will not significantly contribute to healing of muscles, tendons, ligaments, cartilage, or any other tissues.
So If A Heating Pad Is Not The Answer, What Is?
